Community and Facility Based Services

 

Community Access Group

CAG services may be provided in the facility or community.  They are designed to assist the individual in acquiring, retaining or improving self-help, socialization and adaptive skills required for active community participation outside the individual’s home or family home.  CAG services are provided to groups of individuals, with a staff ratio of one to two or more.

 

 

Community Based Services

 

Community Access Individual

CAI services are provided one staff to one individual in a community setting.  These activities could include accompanying an individual to the grocery store, eating establishment, volunteer job, recreational activities, gym, a class, and other community resources, teaching an individual how to participate in appropriate social and recreational activities.

 

In Home and Community Services

 

Community Living Services

CLS services are individually tailored supports that assist with the acquisition, retention, or improvement in skills related to an individual’s continued residence in his or her own or family home.  Services include training and assistance with activities of daily living such as bathing dressing, toileting, personal hygiene, light house work, laundry, meal preparation, transportation, grocery shopping, using the telephone.  CLS services may include medically related services such as basic first aid, arranging and transporting individuals to medical appointments.

Other Services

 

Specialized Medical Supplies

SMS consist of food supplements, special clothing, diapers, bed wetting protective chucks, and other supplies not available under the Medicaid State Plan.

 

Specialized Medical Equipment

SME services include various devices, controls or appliances which are designed to enable individuals to interact more independently with their environment.  This is a service available through the NOW Waiver.  It is used to pay for equipment not covered by the State Medicaid Plan and is the payer of last resort, this must be documented prior to purchase of equipment.
